Humana Partners With Accordant Health Services For Chronic Disease Management; Agreement Will Provide Personal Outreach To Members With Complex, Chronic Diseases.

LOUISVILLE, Ky., July 15 /PRNewswire/ -- Humana Inc. (NYSE: HUM) today announced an agreement with Accordant Health Services, Inc., under which Accordant will provide disease management services to Humana members with complex, chronic diseases like multiple sclerosis, myasthenia gravis, hemophilia, systemic lupus, and cystic fibrosis.

"No one benefits more fundamentally from the advantages of coordinated care than someone who must deal every day with a chronic medical condition," said Gregory H. Wolf, president and chief executive officer of Humana. "Our agreement with Accordant is the latest example of how Humana's team approach to medical care delivery reduces suffering and prolongs the lives of members who need our help the most."

"Aggressive management of chronic conditions can yield profound benefits for our members," said Jerry Reeves, M.D., Humana's chief medical officer. "We're pleased to build on our successful efforts with congestive heart failure, diabetes and end-stage renal disease by offering this innovative program to our members."

Under the program, Accordant case managers will work directly with Humana members who have the targeted diseases, educating them and monitoring their health. By helping these members better manage their own conditions, the program should enable them to lead healthier lives and avoid expensive hospital stays.

Humana has already launched the chronic-disease program in Louisville, Lexington, Ky., and Kansas City. Over the next six months, it will roll out the program in a total of nine states.

Accordant Health Services, Inc., is an independent disease management company based in Greensboro, N.C. The company, founded in 1995, focuses on managing complex, chronic diseases with the goal of improving clinical, patient satisfaction, and cost of care outcomes. At present, the company manages 14 diseases in the areas of neurology, rheumatology, pulmonology, and hematology.
